Reinke’s edema occurs when the layer under the surface lining of the vocal fold - called Reinke’s space - fills with fluid due to long-standing vocal trauma, most often from smoking. In the most severe cases, the entire membranous portion of the vocal folds becomes filled with thick, gelatinous fluid, and the vocal folds look like enlarged fluid-filled balloons/elephant ears. Smokers polyps or Reinke's edema is caused by the combination of smoking and talking a lot. The voice is very deep because of the size of the polyps. You will see them suck inward when the patient takes a breath. After surgical removal, the vocal cords vibrate at a much higher pitch.

A direct laryngosocopy is performed with a patient asleep. Cuts are made in the vocal fold and the excess tissue is removed. Minimally invasive surgery is also possible for Reinke’s edema in which the disease is treated in the office using laser. If Reinke’s edema fails to improve sufficiently following smoking cessation, microlaryngoscopic surgery is the main treatment option. Surgery for Reinke’s edema has unique difficulties, because the superficial lamina propria must be reduced - but not excessively, in order to preserve vibration of the vocal fold.

Reinke edema is one of the common cause of dysphonia middle-aged population, and severe thickening of vocal folds require surgical treatment. Smoking plays a major role on etiology. Vocal fold cysts are also benign lesions and vocal trauma blamed for acquired cysts. We would like to present 3 cases with vocal fold cyst related with Reinke edema.

Most procedures follow the microflap technique set in place by Hirano. During surgery, an incision is made into the vocal cord using either microscissors or a CO 2 laser. Traditionally, Reinke’s edema was treated by performing a direct laryngoscopy in which incisions were made in the vocal folds to remove the excess tissue. Today, microlaryngoscopy, a minimally invasive surgery using a laser, is a much more common technique. Traditionally Reinke’s edema was treated with surgery in the operating room. A direct laryngosocopy is performed with a patient asleep.
